The visiting nurse arrives at the home with a device bag. Setup must happen during the visit, typically within 10-15 minutes of a 30-45 minute total visit. The nurse verifies data is flowing to the monitoring dashboard before walking out the door. If it isn’t flowing, there is no second chance until the next visit (which might be 3-7 days away).
The patient often cannot manage the device independently. Cognitive decline, physical limitations, language barriers, or low health literacy mean the caregiver (spouse, adult child, home health aide) is the actual device user. The caregiver needs to understand one thing: how to confirm the device took a reading. Everything else, trending, alerts, billing, is handled by the agency and the monitoring team.

Which Devices Work Best for Home Health Settings?
The selection criteria, in order of priority for home health: (1) connectivity independence, (2) setup speed, (3) caregiver usability, (4) clinical accuracy, (5) cost.
Clinical accuracy is not first. Every FDA-cleared device in the categories below meets clinical accuracy thresholds. The question is not “does it measure accurately?” The question is “will it reliably transmit data from this patient’s home?”
- Blood pressure cuffs. Cellular first. Tenovi and BodyTrace cellular cuffs transmit without WiFi or a smartphone. The nurse unwraps the cuff, puts it on the patient, presses the button, and watches the reading appear on their tablet dashboard. Three to five minutes. The sensor pairs to the patient’s phone or a relay device and transmits automatically after initial setup. - Medication management. Smart pill dispensers (Hero, MedMinder) are high-value additions for polypharmacy patients, common in home health. The dispenser alerts caregivers and the care team when doses are missed. Setup requires loading medications during the home visit (15-20 minutes for a complex medication regimen).

What Does Device Setup Look Like During a Home Visit?
The 10-minute rule: if the device is not transmitting data within 10 minutes of unboxing in the patient’s home, it is the wrong device for home health.
Pre-configure before the visit.
The agency office should be the setup room, not the patient’s kitchen table. Before the home visit, the nurse or a device coordinator should: unbox the device, power it on, verify it connects to the monitoring platform (take a test reading in the office), label it with the patient’s name and ID, and pack it in the nurse’s device bag ready to deploy.
When the nurse arrives at the home, the device is already configured. The setup consists of: place the device (scale on hard floor, cuff on nightstand, pulse ox on bedside table), take the first real reading with the patient, verify the reading appears on the nurse’s tablet dashboard in real time, and train the caregiver.
Caregiver training, not patient training.
In most home health scenarios, the caregiver is the device manager. The patient wears the cuff or steps on the scale. The caregiver watches for the confirmation (green light, beep) and knows to call the agency if the confirmation doesn’t appear for two consecutive days.
The training covers three things: (1) how to help the patient use the device, (2) what the confirmation light/sound means, (3) who to call if it stops working (the agency, not the device manufacturer’s 800 number). Three things. Five minutes. That is the entire caregiver training.

What’s Different About Home Health Billing vs Standard RPM?
This is where home health agencies often get confused, because the billing structure is different from physician-office RPM.
- Home health agencies bill under Medicare Home Health PPS (Prospective Payment System), a per-episode payment model. The agency receives a lump payment for a 30-day or 60-day home health episode covering all services delivered.
- RPM CPT codes (99453-99458) are physician/practitioner billing codes. The ordering physician or qualifying practitioner bills RPM codes, not the home health agency. This means the home health agency provides the monitoring service (devices, data collection, care coordination) while the physician practice captures the RPM billing revenue.
- The operational model that works: the physician practice orders RPM for the home health patient. The home health agency deploys and manages the devices as part of the home health episode. The physician practice bills RPM codes based on the device data and interactive communication time. Revenue sharing between the physician practice and the home health agency covers the agency’s device and operational costs.
- CCM stacking applies here too. Home health patients with 2+ chronic conditions (most of them) qualify for concurrent CCM (99490). The physician practice bills both RPM and CCM while the home health agency delivers the care coordination. Combined per-patient revenue: $167-184/month from the physician practice, which funds the device deployment the home health agency manages.

The Device That Works in the Patient’s Home Is the Right Device
Vendor demos happen in conference rooms with perfect WiFi, stable Bluetooth, and product managers who know every button. Home health happens in apartments with spotty cell service, patients with tremoring hands, and caregivers learning the device for the first time during a 30-minute visit where the nurse also has to do a clinical assessment, reconcile medications, and document in the chart.
The device that is clinically superior on paper but fails setup in a home visit is the wrong device. The device that measures with slightly less precision but transmits reliably from a rural home with no WiFi and gets used every day by a caregiver who received five minutes of training, that is the right device.
