Which Patients Qualify for Both Programs?

How Do You Track Time Without Double-Counting?
This is the compliance question that stops most billing teams. The answer is simpler than they expect but requires discipline.

The gray area (and how to handle it): A care manager calls a patient for the monthly RPM check-in. The call starts with reviewing blood pressure readings from the past week (RPM time). The conversation shifts to the patient mentioning they missed their endocrinology appointment (CCM time). The care manager spends 5 minutes discussing readings and 10 minutes coordinating the rescheduled appointment.

What Changes in 2026 Affect Stacking?
