A Hospital at Home technology platform is the integrated system of FHIR-grade EHR integration, continuous RPM monitoring, clinical AI for patient selection and documentation, patient-facing communication tools, and HIPAA-compliant data layers that supports an AHCAH-approved Hospital at Home program. It is the software infrastructure that lets a hospital bill traditional Medicare for AHCAH services while meeting CMS operational and reporting requirements.

PL 119-75 Section 6210 extended the AHCAH waiver through September 30, 2030 without changing the operational or technology requirements. What changed is regulatory certainty: a 5-year runway now matches the 18-36 month capital payback window health systems model in their board approval. Yes. Epic Community Connect is the most common EHR configuration across critical access hospitals and smaller regional hospitals piloting H@H, and Mindbowser's FHIR integration layer (HealthConnect CoPilot and EHRConnect) supports Epic Community Connect instances. The integration work includes Epic App Orchard registration, FHIR R4 endpoint configuration, SMART on FHIR OAuth 2.0 flow, and USCDI v3 data element mapping.

The federal AHCAH waiver gives a hospital the regulatory permission to bill Medicare at the inpatient DRG for H@H services. State RHTP funding finances the capital build (FHIR plumbing, RPM device fleet, clinical AI, staffing capacity) for H@H expansion where the state plan named H@H as a fundable activity. The two programs are designed to be additive: AHCAH provides the reimbursement framework; state RHTP provides the build financing. Multiple states (Massachusetts, others) explicitly included H@H expansion in their RHTP plans.
