How Practicing Surgeons Replaced the Endoscopy Tower With an iPad-Based, HIPAA-Compliant IoT Platform

A surgeon-founded team came to Mindbowser with a concept, not a product: a case built around an iPad mini that replaces a full endoscopy tower. We worked as the technology partner from idea through launch, building Bluetooth hardware control, encrypted live video, offline-first storage, and NPI-gated clinician access into one platform.

Customer Focus

A surgeon-founded medtech startup building a portable, cloud-connected endoscopy platform to replace the conventional endoscopy tower.

Scope

End-to-end product build: Bluetooth hardware control, encrypted live video, offline-first storage, NPI-gated access, and a separate web analytics layer.

Stack

iOS (native), React, Python, OpenTok, Bluetooth LE (RFduino), HIPAA-compliant AWS S3

Outcomes

One Integrated Platform, Built From a Concept to a Shipped Device

Mindbowser took the founders' idea for a tower-free endoscopy system and built it end to end, as one iOS application backed by HIPAA-compliant cloud infrastructure.

  • Built what the client's own team describes as a first-of-its-kind HIPAA-compliant healthcare IoT platform for mobile endoscopy.
  • Delivered working telemedicine capability through OpenTok, enabling real-time remote physician supervision during live procedures.
  • Shipped a Bluetooth-controlled hardware interface, offline-first video storage, and NPI-gated access control as one integrated iOS application.
  • The founders went on to raise $3.6M in funding across three rounds following the platform's launch, a business outcome built on top of the product.
The Problem

Four Requirements, One Non-Negotiable Foundation

The platform had to be HIPAA compliant from day one, not retrofitted later. That requirement shaped almost everything else on the list.

01
Real-time hardware control, mid-procedure

Physicians needed to control LED intensity, camera settings, and the pump over Bluetooth, simple enough to use without fumbling through menus mid-scope.

02
Remote second opinions, across platforms

Physicians needed live video conferencing so an attending could supervise a procedure remotely, joining from iOS or web without matching devices or operating systems.

03
Secure storage that survives a dead connection

Patient records needed secure cloud storage with temporary, limited sharing, and the whole system had to keep working in areas with low or intermittent internet, not just fail gracefully.

04
Proving the user is actually a clinician

The real attack surface wasn't Bluetooth pairing or the video codec. It was confirming the person on the other end of a live procedure feed is a licensed clinician, not just someone who found the app.

The Tech Stack

A native iOS app for the hardware and procedure workflow, paired with a separate React and Python web layer the client uses to upload and analyze structured procedure data.

What We Built

Our Approach

A Clean, Bluetooth-Native iOS Interface

We built a professional iOS application with full Bluetooth Low Energy integration through an RFduino microcontroller, giving physicians direct control over LED and pump settings from the same interface they were already using to view the procedure.

  • Full Bluetooth Low Energy integration through an RFduino microcontroller
  • Direct control over LED intensity and pump settings from the same view used during the procedure
  • Built by developers with years of prior Bluetooth protocol experience
  • Designed to avoid lag a surgeon would notice mid-procedure
